You can download and print this prescription. Have your doctor complete the prescription (include date and signature). Then just call us to make your appointment. Bring the completed prescription on your first visit. It's that easy.
In August 2018 Governor Bruce Rauner signed the Direct Access Law.
This means that many patients in Illinois can go directly to an Illinois-licensed physical therapist for evaluation and treatment.
Limitations remain for some healthcare policies.
· Direct Access may be applied to patients in Illinois with the following health plans: United Healthcare, some Aetna, some BCBS, and some Cigna plans.
· Medicare continues to require a signed plan of care as condition of payment and in nearly all instances, a prescription from a licensed physician, physician assistant or nurse practitioner. Medicare Advantage plans fall under Medicare guidelines.
· BPT will not see worker’s compensation or auto injury patients without a prescription.
Patient healthcare policy specifics remain unchanged re: deductible, co-insurance, plan maximum visits, etc. Call your health insurance company regarding your plan requirements in Illinois, as each plan is different.
It is important for patients to note that the law requires physical therapists to report visits to the patient's healthcare professional within five business daysafter the first visit for an injury or ailment.
The patient will also be referred to a doctor under certain specific circumstances.
